Reduced fees are available for care on an income based sliding fee basis.

The sliding fee application must be completed.

Proof of household income may need to be provided.  This may include:

1. A copy of your most recent tax return, W-2s or pay stub(s)
2. A copy of your TANF check, SSI/SSDI check, Retirement Check, VA Benefits, or Banks statements of direct deposit for any of these income sources.
3. Alimony, child support payment, City or General Assistance voucher.
